Terms and Conditions:

1. Aayush Health Card covers 4(four) Members fo the Family. Definition of Family  1.Card Holder 2. His/Her/Spouse  3.Children(Age between 12-21years) or 4 Parents

Aayush health card is applicable to the cash payers only. It is not applicable to credit patients (TPAS/INSURANCE/CGHS/CSMA/ECHS) etc. & those who are already benefitted by any other scheme

2.Aayush health card is not a credit card

3. Kindly present the card at the billing counter before the final bill is made
4. The amount on card is non refundable, no credit is allowed.
5. The management is not responsible for the loss fo the card.
6. Conditions which are not mentioned any, will be at the discretion of management
7. In case of emergency Free Ambulance facility to 'Aayush Hospitals' will be provided (within the city limits) (phone:9603959595)
